Use this form for freight damage, missing parts, or cosmetic/manufacturing concerns. Each image or video must include a written description. Files without descriptions cannot be reviewed.
Freight damage claims require the signed BOL, meaning Bill of Lading, from delivery. The Bill of Lading must show that the damage was written down at the time of delivery. If damages were not written on the Bill of Lading at delivery, the freight carrier may reject the claim. Freight damage claims submitted more than 5 days after delivery are automatically rejected by the carrier.
